Dear patients of Festubert Family Practice,

 

We wish to advise you of a recent change.

 

Medical records will now be stored with Terra Nova Duncan Medical Clinic and can be accessed at 291 Cowichan Way, Duncan B.C. (250) 746-7720. If you are a former patient of Drs. Barr or Cutt waiting to meet the incoming primary care providers, there is NO ACTION required on your part. 

 

Once you start seeing your new provider they will have full access to your medical record.  There will be no charge for this service.   

 

If you wish to obtain or transfer your medical records to a different clinic please contact Terra Nova Duncan Medical Clinic at 250-746-7720.  There will be a fee as this is not an MSP covered service.
